Get An Npi (National Provider Identifier)

If you are a medical provider covered by HIPAA (Health Insurance Portability and Accountability Act), you are required to obtain an NPI (National Provider Identifier.) The NPI is used when filing medical claims, including Medicare. Without an NPI number, you claim will be rejected and your payment delayed. There is no cost to receive an NPI.


Instructions








1. Visit the NPPES (National Plan & Provider Enumeration System) Web site: https://nppes.cms.hhs.gov/NPPES/StaticForward.do?forward=static.instructions


2. Click the link for "National Provider Identifier."


3. Select the option to "Apply Online for an NPI."


4. Click the option at the bottom of the page to "Begin Application Form."


5. Enter the required information. You will need your Social Security number, legacy number, birth certificate information, contact information, license number, EIN and taxonomy code. Submit your application after all information has been entered.


6. Look for your number in the mail. You should receive the NPI number within 15 days of submitting your online application.


7. Contact the NPI enumerator at 800-465-3203 to have the application mailed to you.
